When I create an incident in PagerDuty to ServiceNow, the webhook is getting automatically disabled. Does someone know what the possible reasons could be for webhook getting failed?

When I create an incident in PagerDuty to ServiceNow, the webhook is getting automatically disabled. Does someone know what the possible reasons could be for webhook getting failed?
Common causes are incorrect username/password on the webhooks, significant delay in SN processing the webook and returning a 200 response back to PD.
