Thank you for reaching us out! I just want to let you know that we have identified the cause for the issue with the page loading, I believe we will be able to have that fixed tomorrow.
In the meantime I’m gathering more information about your slack notification concern with other teams. I believe that’s obtained from the incident metadata and, like you said, is also related with the extension.
I’m also looking at the how to remove the extension doubt, I believe the link you shared is the generic way but in this case we should look at this one https://support.pagerduty.com/docs/salesforce-service-cloud-integration-guide#remove-a-salesforce-extension.
I just wanted to share with you some feedback and let you know that we are looking to it, I will return with more information later.

On the removal part, it says to go into Salesforce to remove it, but I’d never successfully connected it to Salesforce so that option doesn’t exist

I have returned from the Christmas break today to find the Salesforce Extension is back, is there a way I can remove it permanently?

Sorry my delayed answer but I returned from holidays today and I only saw your message today.
I believe it’s not possible to configure something like “disabled extensions” or to remove one permanently. I saw that it was added again on December 21, the same day when we removed. Do you know if it was added by someone on your team/company? I saw that you don’t need to configure the integration in order to add it, you just need to select it from the dropdown menu that appears after clicking on “+ New Extension” so that may cause some confusion.
I can remove it again if you want but I would like to check if it is being added by someone by mistake or if it is some automatic process for instance.
How do you prefer to proceed?

Yeah I am guessing someone was “exploring” and clicked it. If you could remove it again that would be great thanks, I will make sure to speak to everyone here to ask them not to do it again
