PagerDuty / ZenDesk integration

I followed Zendesk Integration Guide | PagerDuty to integrate ZenDesk and PagerDuty. I added the ZenDesk extension, authenticate and then by accident I have deleted it. When I tried to add it again I am getting:

  1. “An Admin base role is required.”
  2. Below this error I have a rectangle that state: " Other PagerDuty accounts authorized with this Zendesk account"
    It seems that it “remembers” the previous configuration that was “Deauthorize”.
    Appreciate any help
